Total laparoscopic hysterectomy: the new option

Abstract

Total laparoscopic hysterectomy was done in 6a patients from December 7/1999 to March 24/2001. The average surgical time was 105 minutes. The mean uterine weight was 147 grs with the highest weight being 435 gr. The main clinical diagnosis was uterine leiomyomas (41%), chronic pelvic pain (20%) , cervical squamous intraepithelial lesion (20%) and uterine bleeding (11%). The pathological diagnosis was, principally, uterine leiomyomas (38%), adenomyosis (39%), high-grade and low-grade squamous intraepithelial lesion (16%) and occult infiltrative cancer in one patient (2%). Only in two of the total number of patients was there no uterine histological pathology found (3%). We highlight the use of a uterine manipulator of national invention, and we highly recommend the total laparoscopic hysterectomy as the first option versus the abdominal hysterectomy, in units with enough experience in laparoscopic surgery.
