Evaluación nutricional en cirugía ginecológica

Se evaluaron 28 pacientes de cirugía ginecológica electiva con una edad promedio de 47 años. Se valoró su estado nutricional utilizando los denominados parámetros nutricionales:1. Compartimiento Somático: relación peso/talla, espesor del pliegue cutáneo del triceps, circunferencia muscular del brazo e índice de creatinina talla.
2. Compartimiento Visceral: Albúmina senca, transferrina sérica y conteo absoluto de linfocitos en sangre periférica. Las pacientes se evaluaron preoperatoriamente con su evaluación pre-quirúrgica de rutina. Se consideraron varias complicaciones post-operatorias: sepsis, fístulas, peritonitis, muerte, neumonía, infección de la herida, deshicencia de la misma, falla hepática o respiratoria, shock, flebitis. Se calculó el riesgo de morbilidad y mortalidad operatoria utilizando el índice predictivo de Buzby.
Se comparan dos grupos de pacientes: complicadas Vs. no complicadas. Se analizan los resultados con metodología. estadística de T de Students y Chi cuadrado.
Se complicaron cuatro pacientes para un 14% de complicaciones. 13 pacientes presentaron un estado nutricional adecuado y 15 diferentes tipos de desnutrición equivalente al 53.57%. El nivel de albúmina sérica presentó diferencia estadísticamente significativa en los dos grupos P menor de 0.04. Las pacientes no complicadas presentan superavit de albúmina, linfocitos y transferrina. El índice de Buzby fue diferente para los dos grupos con P menor de 0.035.
