Estudio sobre paso de células fetales a la circulación materna en 461 señoras durante el embarazo y en 1.002 mujeres durante el parto

Resumen

Se estudiaron 1.002 señoras con sus respectivos hijos para valorar el paso de células fetales hacia la circulación materna en el momento del parto. Encontrándose 171 niños que sangraron hacia la madre, lo cual representa 17,06%.

La hemorragia feto materna osciló de 0,10 ml. a 200 mi. y su valoración se hizo por medio de la técnica de Kleihauer modificada.

Se presentaron tres hemorragias de gran magnitud (200 mi., 177 mi. y 136 mi.) naciendo los niños con verdaderos signos y síntomas de shock por anemia aguda; los cuales se recuperaron con tratamiento a base de transfusiones. La paciente que recibió la sangría de 200 mi. presentó en su circulación una supervivencia de células fetales de 48 días.

Se analizaron 23 señoras Rh negativas (con hijo Rh positivo) en las cuales se buscaron anticuerpos en el 1º, 3º y 6º mes postpartum. Cuatro de ellas presentaron en su circulación células fetales y en las 19 restantes no se encontraron. En las que tuvieron paso de eritrocitos una desarrolló anticuerpos anti Rh al 6º mes; y en las que no tuvieron hemorragia feto materna una también desarrolló anticuerpos al tercer mes postpartum.

Total de madres Rh negativas con hijos Rh positivos en los 1.002 casos estudiados 76 (7,6%). Con incompatibilidad de grupo entre madre e hijo 345 (34,5%).

El análisis electroforético de hemoglobina en 1.002 niños arrojó 1,9% de hemoglobinas anormales:14 ni

ños con rasgo Bb. S y 5 con rasgo Hb. C. El promedio de Hb. en las madres con H. F. M. fue de 11,74 ± 1,49 gms.% y en los hijos 16,71 ± 1,82 gms.%.

Se estudiaron a la vez 471 señoras del 5? al 9? mes de embarazo encontrando H. F. M. en 41 (8,87%).
